Attend one of the free WIN for Alaska, Inc. biometric screening events to obtain your biometric data, including: weight, height, body composition, blood pressure, waist measurement, total cholesterol and glucose.

Register online for your 15-minute biometric screening appointment. Fasting and non-fasting screening appointments are available.

To view, print and post a schedule of the biometric screenings in your area, click on your corresponding campus link below.

At your screening event, enter to win a Garmin Forerunner 410 GPS Heart Rate Monitor, nearly a $300 value! One per major campus (UAA, UAF, UAS). The prize drawing will be held on May 5, 2012 and the winners will be contacted at that time.

Remember to enter your screening results into Get the Point for points toward your Get the Point goal.

JHCC $50 Reimbursement
To participate in the UA Joint Health Care Committee (JHCC) Reimbursement Program, please download and complete the Health-related Reimbursement Form and include it with your proof of purchase information. Proof of purchase items must be dated between January 1, 2012 and April 30, 2012 to be eligible for reimbursement.

Envelopes must be postmarked by April 30, 2012 to be eligible. Emails and faxes will not be accepted.

You must be enrolled in the UA Choice health plan to be eligible.






Personal Wellness Profile
The Personal Wellness Profile is a brief online health questionnaire that will give you a confidential report of your potential health risks based on your answers. The report will offer a summary of recommendations and resources to help guide you toward better health.

The Personal Wellness Profile will now be available year-round beginning April 1, 2012. To access the registration link, visit www.uahealthinaction.com and look for the Know Your Numbers section, or go to the Personal Wellness Profile section

Only UA Health Plan eligible employees and spouses or FIPs may take the PWP.
