Personal Wellness Profile
Available year-round

The Personal Wellness Profile is a brief online health questionnaire that will give you a confidential report of your potential health risks based on your answers. The report will offer a summary of recommendations and resources to help guide you toward better health.

Remember to enter that you've completed your Personal Wellness Profile into Get the Point for points toward your Get the Point goal.
